PlatinumStripe® 1800 PE-RT Project Request Form

Thank you for your interest in Performance Pipe, a division of Chevron Phillips Chemical Company LP, and our PlatinumStripe® 1800 PE-RT pipe and fittings. 

Properly submitted project request will be evaluated, and at its sole discretion, Performance Pipe will determine whether to issue a quote to requestor.

Submission of this Project Request Form does not entitle requestor to purchase PlatinumStripe® 1800 PE-RT pipe and fittings products and does not obligate Performance Pipe to issue any quote or sell any product to requestor. Requestor submitting this Project Request Form does not prohibit Performance Pipe from selling PlatinumStripe® 1800 PE-RT pipe to any other buyer.

  • If multiple requests for quote are received for any one project, Performance Pipe reserves the right to select and quote only one requestor for each project.
  • Performance Pipe will arrange freight for all domestic deliveries.
  • For any issued quote, Performance Pipe will, for a period of six months following the date on which such quote is issued, appoint requestor as the exclusive distributor for sales of PlatinumStripe® 1800 PE-RT pipe and fittings for the quoted project.
  • Requestor agrees that submission of this Project Request form, either by (i) clicking "Accept and Submit" below or (ii) emailing to any of the addresses listed above, is a representation that all of the information provided is accurate and that requestor agrees that any sale of PlatinumStripe® 1800 PE-RT will be subject to and governed by Performance Pipe's standard terms and conditions of sale as found at this link, notwithstanding any other provisions contained in any purchase order, acknowledgment, acceptance or other document of requestor.
